Forms 1, 1A (individual or team), 1B, and an abstract are REQUIRED for all JUNIOR and SENIOR division projects. These forms should be placed in a blank folder (no names) in front of the project on the day of the fair. DO NOT SEND THESE FORMS TO THE FAIR DIRECTOR!!!
If a junior or senior division project involves human subjects (including surveys), non-human vertebrate animals, pathogenic agents, controlled substances, recombinant DNA, human or non-human vertebrate animal tissue or is conducted in a registered research institution (e.g. university laboratory, medical center, etc.) or industrial setting, additional forms are required.
