• $30.00 Application Fee (check or money order only) - Please make out to Louisiana Tech University (Due only if you are not a current Tech student).
• Louisiana Teaching credentials, obtained from teachlouisiana.net or a copy of your official Teaching Certificate
• Selective Service Form
• Immunization Compliance Form (Request for Exemption is all that is necessary if your records are not accessible)
• Current telephone number and email address
