The graduate school process is replete with forms that allow the Graduate School to function properly. Important forms and curriculum sheets are listed below. See the LEC Doctoral Program Handbook and the Psychology Department Ph.D. Student Handbook for additional information on the doctoral programs.
The forms are all in Adobe PDF form. On almost all the forms you can type in much of the information. Simply click on the area you wish to edit, and if it is a typable field, the cursor will change to a vertical bar. If it does not change you will have to fill in the information manually.
If you are unable to download the handbook or forms, you can pick them up from the Graduate Studies office located in Woodard Hall 103.

Course Substitution Approval Form
This must be signed by your advisor and others if you wish to request a course substitution. The request must be submitted prior to enrollment in any course that deviates from the signed Plan of Study.
Transfer Course Approval form
Complete this form if you want to request the transfer courses (already taken) from another college or university.
Off-Campus Course Approval form
Complete this form with your advisor for the purpose of transferring coursework (not taken yet) from another college or university to your plan of study at Louisiana Tech University.
Change of Program Request form
This form must be completed, along with a new application and new Plan of Study, when a student wishes to change their program of study.
