Application Guidelines

Please read through these guidelines carefully before beginning the registration process. Doing so will give you a better understanding of the registration process, and may prevent you from accidentally skipping a key part of the registration process.   • Please make sure that all information on your application form is accurate. This includes addresses, e-mail addresses, telephone numbers, and other contact information. Submission of incorrect data will negatively impact your chances of being assigned to your chosen camps.
  • Please only choose camps that you are sure you can attend. If you apply to and are assigned to a camp you cannot attend, it may not be possible to move to another camp.
  • LA GEAR UP will make every effort to assign you to one of your three choices of camps. However, space is limited and camp slots are assigned on a first-come-first-serve basis. If none of your chosen camps are available, you may be assigned to another camp.
  • Online application is recommended as it helps save paper, is more efficient, and places you in the application system immediately.
  • Online application DOES NOT IMMEDIATELY REGISTER YOU FOR A CAMP. Upon application you will be placed on a waiting list for assignmentuntil your entire application package is completed. YOU WILL NOT BE REGISTERED FOR CAMP UNTIL ALL OF YOUR REGISTRATION DOCUMENTS HAVE BEEN SUBMITTED.
  • ONLY WHEN YOUR APPLICATION PACKAGE IS COMPLETE WILL YOU BE ASSIGNED TO A CAMP.
  • For legal reasons, even if you register online, you MUST STILL SUBMIT SIGNED COPIES of the "Parent/Guardian" form and the "Release, Waiver, and Authorization for Medical Treatment" forms found in your application packet.
  • Even if you register online, you MUST STILL SUBMIT COPIES of the signed "Recommendation Form" and the "ACES Survey" found in your application packet.
  • DOCUMENTS SHOULD BE SUBMITTED TO YOUR CLUB SPONSOR! Do not send application forms directly to the LA GEAR UP office, as this may delay the processing of your application.
  • The application deadline is 15 May 2009, and no applications will be accepted or completed past that date. It is strongly recommended that you have your application packet complete and submitted by 30 April 2009.
