Application for Graduation Form
You must complete an Application for Graduation and turn it in to the University Registrar’s Office before Friday of the third week of the quarter you intend to graduate. This initiates the degree audit process and begins the records preparation and diploma ordering process to prepare for your graduation. If you miss the deadline, see your Academic Dean for their determination as to whether you can be added to the degree-candidate list late. Their deadline is no later than Friday of the fifth week of the quarter.
You must also order a cap and gown at the University Bookstore. This must be accomplished prior to Friday of the sixth week of the quarter you intend to graduate.
The Application for Graduation will be sent to your academic dean’s office to inform them that you expect to complete your degree requirements this quarter. Your academic record will be reviewed along with your degree plan (plan of study). If there is a problem, your academic department or dean’s office staff will contact you. Please see the University Catalog under which you are graduating to review academic and residency requirements for associate, baccalaureate, masters, and doctoral degrees. The catalog also contains the criteria for conferral of the baccalaureate degree with Latin honors.

If you will be taking a course at another school during your final quarter, we will need to have verification from that school of your final grade in the course before you will be allowed to participate in commencement ceremonies. You will be allowed to participate in the graduation ceremony ONLY if your dean’s office certifies you as eligible to graduate. Further, we must receive the official transcript from the other school before you can obtain your diploma.
