Louisiana Tech University - Student Affairs::How to Create a Student Organization

Please review the following steps required to ensure your Student Organization (whether new or renew) is processed in a timely manner.

Organizations are required to provide the following to Barry Morales in Student Activities (on campus in Tolliver Hall) by Friday, October 13^th, 2006 in order to be officially recognized by the University.

A list of ALL current members along with their phone numbers
A list of ALL officers/advisors along with their phone numbers and email addresses
A schedule of your organization's meeting times, dates, and locations
The Student Organization Contract (pdf) must be signed by ALL authorized signatures
The Student Organization Hazing, Alcohol, & Substance Abuse Policy (pdf) must be completed
The Student Organization/Faculty Advisor Relationship Agreement (pdf) must be completed
The Student Organization Information Sheet (pdf) must be completed
The Student Organization Model Constitution And Check List (pdf)
The Student Organization Sample Constitution (pdf)
Constitutional Revisions/Changes*

*If you have made revisions in your constitution since last year, the changes must be submitted as well for approval by the Student Organizations Committee. Any new organization must also include a copy of its constitution and by-laws with its registration packet.
