Address and Name Changes

Each student is responsible for updating any changes to their address, name, and/or contact information as soon as possible after the change occurs.  There are several methods available for use.

PLEASE NOTE: this process is only to be used for currently enrolled students attending through Summer Quarter, 2024.  Students attending Fall Quarter, 2024 and forward will initiate Address and Name Changes in Workday Student.

Online:

Go to your BOSS account, menu options “Addresses”, “Personal Phone Numbers”, and “E-mail Addresses” to instantly update your contact information.

 

By USPS Mail:

Open, print, and complete the Request for Address Change and/or Request for Name Change PDF form.  Sign it, and mail it to:
