2017-2018 Documents and Forms

At times, the Office of Financial Aid requires information in addition to the information collected through the FAFSA to completely and accurately process your financial aid package. We will notify you if additional information is required, or you can periodically check your missing documents on your Net Partner account.

Be certain that you are using the correct form for the correct award year.  Submitting forms that are NOT requested by our office can delay the processing of your financial aid.

A

Academic Degree Completion Plan Adobe PDF icon (ACTION REQUIRED BY STUDENT): This form is required for high aggregate hours Satisfactory Academic Progress (SAP) appeals.  The faculty advisor will list courses that are required for the student to complete a degree and the expected graduation date.  The student and advisor must sign the form before submitting it with the appeal. Academic Transcript Review - Transfer UG (ACTION BY THE UNDERGRADUATE STUDENT)Once all final, official post-secondary academic transcripts are received by Admission’s and/or the Registrar’s Office and posted on the student’s B.O.S.S. transcript screen, they will be reviewed by the Financial Aid Office for eligibility. If you are determined eligible based on the Louisiana Tech University Satisfactory Academic Progress Policy, you will be awarded aid if all other required documents are received. If you are determined ineligible, you will be notified by email of your appeal and/or reinstatement options.

Academic Transcript Review - Transfer GR (ACTION BY THE GRADUATE STUDENT): Once all final, official post-secondary academic transcripts are received by Admission’s and/or the Registrar’s Office and posted on the student’s B.O.S.S. transcript screen, they will be reviewed by the Financial Aid Office for eligibility. If you are determined eligible based on the Louisiana Tech University Satisfactory Academic Progress Policy, you will be awarded aid if all other required documents are received. If you are determined ineligible, you will be notified by email of your appeal and/or reinstatement options.

Academic Transcript Review - Dual (NO ACTION BY THE STUDENT): Since you are a new freshman and the academic transcripts (college credits) were earned while still in high school, the document will be temporarily bypassed in order for you to be awarded aid. However, your estimated aid will not appear on your student account as a credit until all college academic transcripts have been received and posted. You will need to ensure that all official academic transcripts are sent to Louisiana Tech University as soon as possible.

Access Code Form (STUDENT INITIATED ACTION): This form is used to establish a Financial Aid Access code if you do not have one.  The access code is required for logging into the MY AID website and is requested by the Financial Aid staff when specific financial aid information is asked by the student/parent/other over the phone or in person regarding their file.

Active Duty Military (copy of orders) (ACTION REQUIRED BY STUDENT): This document is required because the student answered "yes" that they are currently serving on active duty in the U.S. Armed Forces for purposes other than training on the FAFSA.  This "yes" is the only thing making the student independent for financial aid purposes.

Address Change Form Microsoft Word icon (STUDENT INITIATED ACTION): This form is used to change the student's address on their financial aid record.  Please be aware that this does NOT change the address with other offices on campus (i.e. Registrar's Office). 

Adoption Legal Document (STUDENT INITIATED ACTION): This document is required because the student has stated they have been adopted.  The legal court document is required to be submitted as proof.

Amended Tax Return-Parent (ACTION REQUIRED BY STUDENT): This document is required to complete the verification process because changes were made by the parent on the initial tax return.

Amended Tax Return-Spouse (ACTION REQUIRED BY STUDENT): This document is required to complete the verification process because changes were made by the student's spouse on the initial tax return.

Amended Tax Return-Student (ACTION REQUIRED BY STUDENT): This document is required to complete the verification process because changes were by the student on the initial tax return.

Application for Hardship Waiver (ACTION REQUIRED BY STUDENT): Louisiana Tech University has established a hardship waiver program to provide limited, partial tuition/fee assistance to its most needy undergraduate students who have utilized all available federal financial aid programs and still have significant unmet financial need resulting from the Legislative approved fee increase. Students who meet all of the conditions may apply for a Hardship Waiver for up to three quarters; Fall 2016, Winter 2016-2017, and Spring 2017. Email notices will be sent to students who might be eligible when the form is available on the web.

Aviation Flight Costs Budget Increase Form Microsoft Word icon (STUDENT INITIATED ACTION): Student has indicated to the Financial Aid Office that he/she has Aviation as a major and will have additional costs for flight fees. 

Award Revision Req OB by Fin Aid Office (NO ACTION REQUIRED BY STUDENT): This document will be removed by the Financial Aid Office once award information has been reviewed.  If additional documentation/information is required from the student, they will be contacted by email.

Bankruptcy Papers (copy) (ACTION REQUIRED BY STUDENT): This document is required because the National Student Loan Database System (NSLDS) indicated that the student has one or more loans in active bankruptcy.  The Financial Aid Office needs the portion of the bankruptcy papers that lists all creditors that are included in the bankruptcy.

Birth Certificate (copy) (ACTION REQUIRED BY STUDENT): The date of birth or Social Security Number that you reported on your FAFSA does not match the information in the Social Security Administration's records.  The Financial Aid Office needs to confirm the accurate information and a copy of your birth certificate is required.

BPCC Bridges to Bulldogs Program (ACTION REQUIRED BY STUDENT): Student is enrolled in the Bridges to Bulldogs Program for the Summer 2016 Quarter.  After the student has successfully completed the program and is admitted to the University unconditionally, the document will be removed and the student will be packaged.

Bulldog Out-of-State Scholarship Application Adobe PDF icon (ACTION REQUIRED BY STUDENT): An undergraduate student who is a currently enrolled or readmission student may apply for the Bulldog Out-of-State Scholarship if they meet certain criteria.  Those students who are new freshman or transfers are automatically considered through there application for admission.

Child Care Costs Budget Increase Form Adobe PDF icon (ACTION REQUIRED BY STUDENT): Student has indicated to the Financial Aid Office that he/she has will incur child care costs for a dependent child during the academic year. 

Clear Aggregate Loan Limit (ACTION REQUIRED BY STUDENT): ACTION REQUIRED BY STUDENT - Based upon data provided by the National Student Loan Data System (NSLDS), your grade level, and your dependency status, you may have received student loans in excess of loan limits established for the federal loan program. In general, students who borrow in excess of aggregate loan limits are ineligible to receive further Title IV assistance. However, if the school determines that the student inadvertently borrowed in excess of the limits, the student may regain eligibility either by repaying the amount borrowed in excess of the aggregate limits or by making satisfactory (to the loan holder) repayment arrangements for the excess.

Clear Aggregate Loan Limit Reaffirmation Letter (ACTION REQUIRED BY STUDENT): Based upon data provided by the National Student Loan Data System (NSLDS) and your grade level, we have determined that you may have received a total amount of students loans (undergraduate and graduate) that exceeds the loan limits established for the federal loan programs.  However, you may have reaffirmed the excess debt with your servicer, restoring your eligibility to participate in the federal student aid programs.

Clear Date of Death with SSA (ACTION REQUIRED BY STUDENT): The FAFSA has indicated that the student's social security number (SSN) belongs to a deceased person.  The student should contact the Social Security Office to resolve the conflict or make a correction to the SSN on the FAFSA.

College Work Study Employment Certification-1242 (ACTION REQUIRED BY STUDENT): This form is used to certify student workers who were awarded the Federal Work Study Program on their award letter. FWSP certifications are ONLY available in the Financial Aid Office. Department and student complete the form together and return to the Financial Aid Office with remainder of certification packet.

Complete Highlighted Areas (ACTION REQUIRED BY STUDENT): The Financial Aid Office has received incomplete paperwork.  This paperwork has been sent back to you with highlighted fields.  Please complete the highlighted fields and resubmit.

Confirm Admission Status to LA Tech Univ (ACTION REQUIRED BY STUDENT): This document will appear when your admission status is not showing for the current quarter.  If you are NOT currently enrolled at Louisiana Tech and have not applied for admission/readmission, you will need to check with the Admissions Office regarding your application.  If you have received a letter from the Admissions Office confirming admission, please provide a copy of your admission letter to our office or email our office at techaid@latech.edu.  If you are currently enrolled at Louisiana Tech, please contact our office so that we may clear your status.

Confirm Resources Form Adobe PDF icon (ACTION REQUIRED BY STUDENT): This form must be completed to clarify your asset worth (and your parents', if dependent OR your spouse's, if married).  You may have left the asset questions #41, #42, or #43 blank in the student financial information OR you may have left the asset questions #90, #91, or #92 blank in the parent financial information of the FAFSA OR we may need clarification on these asset information questions due to conflicting information reported on the FAFSA.

La Tech Financial Aid Data Form Adobe PDF icon (ACTION REQUIRED BY STUDENT): By completing the FAFSA and the Louisiana Tech Data Form, you are automatically considered for federal aid. The form is also needed for information pertaining to the Louisiana TOPS scholarship. 

Death Certificate (copy) (ACTION REQUIRED BY STUDENT): This document is required to verify the death of a deceased relative or other family member.

Dependent’s Birth Certificate (copy) (ACTION REQUIRED BY STUDENT): This document is required to complete the verification process in order to verify that the student has a dependent child.

Direct Loan Grad PLUS Entrance Counseling (ACTION REQUIRED BY STUDENT): Once you've accepted a Federal Graduate PLUS loan, you must complete the Graduate PLUS loan entrance counseling and learn about the terms and conditions of your loans.  Students will need to complete counseling if they have never participated in a federal PLUS loan program or have transferred to Louisiana Tech University from another school where they had a federal loan.  The Entrance Counseling must be completed before the loan will be sent to the Department of Education for processing.

Direct Loan Grad PLUS Master Promissory Note (ACTION REQUIRED BY STUDENT):  Once you've accepted a Federal Graduate PLUS Loan, you must complete the PLUS Master Promissory Note (PMPN) in order for the loan to guarantee and disburse.  When you sign the PMPN, you are confirming that you understand that your school may process new loans on your behalf for the duration of your education (up to 10 years) without having you sign another PMPN. You are also agreeing to repay the US Department of Education all loans made to you under the terms of the PMPN. Therefore, it is very important that you completely read and understand all of the information before you sign it.

Direct Loan Master Promissory Note (MPN) Adobe PDF icon (ACTION REQUIRED BY STUDENT): Once you've accepted a Federal Stafford Loan, you must complete the Master Promissory Note (MPN) in order for the loan to guarantee and disburse.  When you sign the MPN, you are confirming that you understand that your school may process new loans on your behalf for the duration of your education (up to 10 years) without having you sign another MPN. You are also agreeing to repay the US Department of Education all loans made to you under the terms of the MPN. Therefore, it is very important that you completely read and understand all of the information before you sign it.

Direct Loan PLUS Credit Check (Parent or Grad Stud) (ACTION REQUIRED BY STUDENT): To take out a PLUS Loan for the first time, you, if you are a graduate student, or your parent, if you are a dependent undergraduate student, must complete a PLUS Application and master promissory note (PMPN). To be eligible for a PLUS Loan, you must not have an adverse credit history, which the Department will check for when you/parent apply for the loan. If you/parent are found to have an adverse credit history, you/parent may still borrow a PLUS Loan if you get an endorser who does not have such a history. The PLUS credit checks are good for 90 days from the application date.

Direct Loan Stafford Entrance Counseling (ACTION REQUIRED BY STUDENT): Once you've accepted a Federal Stafford loan, you must complete loan entrance counseling and learn about the terms and conditions of your loans.  Students will need to complete counseling if they have never participated in a federal loan program or have transferred to Louisiana Tech University from another school where they had a federal loan.  The Entrance Counseling must be completed before the loan will be sent to the Department of Education for processing.

Direct Loan Stafford Exit Counseling (ACTION REQUIRED BY STUDENT): Any student who borrowed a Stafford Loan at Louisiana Tech University is required by law to complete a Stafford Loan Exit Interview if, at any time, the student is not enrolled at least half-time, graduates, or leaves the University (resignation, unofficial withdrawal, transferring to another school, etc).

Disability-Borrower's Acknowledgement (ACTION REQUIRED BY STUDENT): The National Student Loan Data System (NSLDS) indicates that one or more of your federal loans have been discharged. If you have questions, contact the Financial Aid Office.

Disability-Physician's Certification (ACTION REQUIRED BY STUDENT): The National Student Loan Data System (NSLDS) indicates that one or more of your federal student loans have been discharged. If you have questions, contact the financial aid office at your school.

Dislocated Worker Status (document) Adobe PDF icon (ACTION REQUIRED BY STUDENT): This documentation is required because you indicated that you or someone in your family is a dislocated worker. 

Divorce Decree (copy) (ACTION REQUIRED BY STUDENT): This document is required to complete verification because the student or parents have separated since the FAFSA was completed or there is a discrepancy in marital status.

Drug Conviction Question Blank on FAFSA (ACTION REQUIRED BY STUDENT): You left Item 23 blank on your FAFSA. Your failure to provide an answer to this question makes you ineligible to receive federal student aid. Either indicate that you have not been convicted of possessing or selling illegal drugs for an offense that occurred while you were receiving federal student aid (such as grants, loans, and workstudy), or use the worksheet to determine your answer to this question. You can answer Item 23 or your FAFSA or you can correct this item by calling 1-800-433-3243 or by going to www.fafsa.gov. Please understand that a drug conviction does not necessarily disqualify you from receiving student aid.

Drug Related Hold-Denied Aid (ACTION REQUIRED BY STUDENT): Your FAFSA cannot be processed because of issues related to the Anti-Drug Abuse Act of 1988. To address these issues, you must contact CPS at 1-202-377-3889.


E

Emancipation Court Judgement (copy) (ACTION REQUIRED BY STUDENT): This document is required because the student reported that he/she was an emancipated minor on the FAFSA.

Employee Withholding Exemption Certificate (L-4) Adobe PDF icon (ACTION REQUIRED BY STUDENT): The Louisiana state tax withholding form is required to be completed and submitted when a student is becoming certified to work at Louisiana Tech University as a student worker. It is part of student worker certification packet. 

Enrolled in an Ineligible Degree Program (ACTION REQUIRED BY STUDENT): This document will appear when your major shows you enrolled in an ineligible degree program for the current quarter.  You will not be eligible for Federal Financial Aid if you are enrolled in one of these programs.  If you are NOT currently enrolled at Louisiana Tech and this is not the correct major, you will need to check with the Admissions Office regarding the program that you were admitted under.  If you ARE currently enrolled at Louisiana Tech and have begun classes, you will need to check with the Registrar's Office if the program is an error or submit a major change form. Programs include:  •VIST (Visiting Undergraduate)  •VISH (Visiting Undergraduate High School Student)  •VISM (Visiting Undergraduate - Murphy Oil)  •ICPU (ICP Undergraduate)  •ICPG (ICP Graduate)  •LLL (Life Long Learning)  •LLLM (Life Long Learning - Murphy USA)  •DDVI (Dynamics of Domestic and Family Violence/Graduate Post-Bacc Certificate program)  •TEWC (Technical Writing and Communications/Graduate Post-Bacc Certificate program).  •PBGM (Post Baccalaureate Undergraduate - Murphy USA)  •PBUG (Post Baccalaureate Undergraduate)* - Exception - see Preparatory Coursework Certification document description.

Enrollment History Check (FinAid) (NO ACTION REQUIRED BY STUDENT): This document will be removed by the Financial Aid Office once enrollment history information has been reviewed.  If additional documentation/information is required from the student, they will be contacted by email.

Father's 2015 IRS Tax Account Transcript (ACTION REQUIRED BY STUDENT): This document is required to complete verification and may be requested from the IRS online, by telephone, or by completing a paper form. The Tax Account Transcript provides basic information, including marital status, type of return filed, AGI, taxable income, and later adjustments, if any. Note: Tax filers can request a transcript, free of charge, of their 2015 tax return from the IRS in one of three ways.

Father's 2015 IRS Tax Return Transcript (ACTION REQUIRED BY STUDENT): This document is required to complete verification and may be requested from the IRS online, by telephone, or by completing a paper form.  The Tax Return Transcript provides most line items from your original return. Note: Tax filers can request a transcript, free of charge, of their 2015 tax return from the IRS in one of three ways.

Father's 2015 Tax Return (signed) (ACTION REQUIRED BY STUDENT): This document is required to complete the verification process because the student's parents filed separate tax returns.

Father's 2015 W2 (copy) (ACTION REQUIRED BY STUDENT): This document is required to complete the verification process in order to verify wages earned by the Father.

Father's Social Security Card (copy) (ACTION REQUIRED BY STUDENT): This document is required due to a discrepancy with the Father's social security number.  Please ensure that the copy is clear and readable.

Father's U.S. Birth Certificate (copy) (ACTION REQUIRED BY STUDENT): This document is required due to a discrepancy with the Father's date of birth.  Please ensure that the copy is clear and readable.

Federal Student Aid Report (FAFSA) (STUDENT INITIATED ACTION): This status of this documents indicated whether or not the Financial Aid Office has received your Free Application for Federal Student Aid (FAFSA). You may complete your FAFSA online at http://fafsa.ed.gov. Please complete any other forms that are required to complete your Louisiana Tech application process.

Federal Work Study Job Application (ACTION REQUIRED BY STUDENT): This document is used to assist in assigning eligible Federal Work Study Program student workers to potential employers in Fall Quarter.

Financial Aid ISIR Corrections Pending (NO ACTION BY STUDENT): The Financial Aid Office is waiting for corrections to be processed on the ISIR.

Foreign National Data Collection Form Adobe PDF icon (ACTION REQUIRED BY STUDENT): This form is used to collect required International student data such as immigration status, log of visits to U.S., and to determine if/when a student has met the guidelines for declaring their own tax withholdings versus the standard $33.10 withholding of Federal tax. Student must attach copies of supporting documents listed in the top paragraph on form. It is part of student worker certification packet. 

Form 1-9, Employment Eligibility Verification (ACTION REQUIRED BY STUDENT): This is a Federal form used for employment eligibility verification. Student must attach copies of supporting documents used in the verification of employment eligibility. Student completes Section 1, department completes Section 2. It is part of student worker certification packet.

Form W-4 (2016) Adobe PDF icon (ACTION REQUIRED BY STUDENT): The Federal tax withholding form is required to be completed and submitted when a student is becoming certified to work at Louisiana Tech University as a student worker. It is part of student worker certification packet. 

FSEOG Overpayment-Cleared (Dept of Ed) (ACTION REQUIRED BY STUDENT): To resolve your FSEOG overpayment, call the U.S. Department of Education at 1-800-621-3115, or write to the U.S. Department of Education, P.O. Box 5609, Greenville, Texas  75403-5609.

Guardianship Legal Document by Agency (ACTION REQUIRED BY STUDENT): This document is required because the student reported that he/she was in legal guardianship on the FAFSA.


H

Head of Household Adobe PDF icon (ACTION REQUIRED BY STUDENT): This document is requires because of the way the parent(s) filed their 2015 IRS Tax Return. The Department of Education requires Financial Aid administrators to verify your parents' tax filing status. 

High School Completion Status Document (ACTION REQUIRED BY STUDENT): This document is required to complete the verification process because the student did not report or there is a discrepancy in their high school completion status. Provide one of the following documents:  a copy of the student’s high school diploma; a copy of the student’s final official high school transcript that shows the date when the diploma was awarded; a copy of the student’s General Educational Development (GED) certificate or GED transcript; an academic transcript that indicates the student successfully completed at least a two-year program that is acceptable for full credit toward a bachelor’s degree; if State law requires a home-schooled student to obtain a secondary school completion credential for home-school (other than a high school diploma or its recognized equivalent), a copy of that credential; if State law does not require a home-schooled student to obtain a secondary school completion credential for home-school (other than a high school diploma or its recognized equivalent), a transcript or the equivalent, signed by the student's parent or guardian, that lists the secondary school courses the student completed and documents the successful completion of a secondary school education in a homeschool setting, or If the student is unable to obtain the documentation listed above, he or she must contact the financial aid office.

Homeless Documentation by Agency (ACTION REQUIRED BY STUDENT): This document is required because the student reported that he/she is either homeless or is self-supporting and at risk of being homeless.


I

I-94,I-151 or I-551 Res Alien Cd (copy) (ACTION REQUIRED BY STUDENT): The student did not indicate on the FAFSA that they were a U.S. Citizen or eligible non-citizen.  The student must provide proof of the Alien Registration number if appropriate or make a correction on the FAFSA.  Students in the U.S. on an F-1 Visa are NOT eligible for federal financial aid.

Intent to Return Form (ACTION REQUIRED BY STUDENT): Students who enroll in Summer modules (3 week or 6 week courses) must complete this form if they are dropping a course during the Summer, but intend on attending future modules. Summer Federal Financial Aid is awarded based upon enrollment. Failure to complete this form may result in the need for a Return of Title IV Funds (RT4F) calculation. Failing to enroll and attend in the courses you have indicated on this form may also result in RT4F calculation.

Iraq & Afghan Overpayment-Cleared (DOE) (ACTION REQUIRED BY STUDENT): To resolve your Iraq/Afghanistan Service Grant overpayment, call the U.S. Department of Education at 1-800-621-3115, or write to the U.S. Department of Education, P.O. Box 5609, Greenville, Texas 75403-5609.

La Tech Financial Aid Data Form Adobe PDF icon (ACTION REQUIRED BY STUDENT): By completing the FAFSA and the Louisiana Tech Data Form, you are automatically considered for federal aid. The form is also needed for information pertaining to the Louisiana TOPS scholarship. 

Loan Default-Cleared (letter) (ACTION REQUIRED BY STUDENT): The National Student Loan Data System (NSLDS) indicates that you are in default on one or more federal student loans. You are not eligible to receive any federal student aid until you resolve any loan default(s). Depending on the loan status, you will need to contact GA, DLS, or EDR and make satisfactory arrangements to repay the loan(s). If you have repaid the loan(s), obtain documentation that the loan identified as being in default is the loan that was paid off.

Loan Fraud-Cleared (letter) (ACTION REQUIRED BY STUDENT): The National Student Loan Data System (NSLDS) indicates that you have one or more student loans that may have been obtained fraudulently. You are not eligible to receive any federal student aid until this issue is resolved. You should contact the loan holder of your loan for resolution.


M

Major is Visiting High School Student (ACTION REQUIRED BY STUDENT): The current listed major is Visiting High School Student.  This status does not qualify for Federal Aid.  Federal Aid is only awarded to degree seeking students.

Marriage License (copy) (ACTION REQUIRED BY STUDENT): This document is required to complete verification because the student or parent has married since the FAFSA was completed or there is a discrepancy in marital status.

Missing Page of Tax Return Transcript (ACTION REQUIRED BY STUDENT): When you submitted your Tax Return Transcript a page was missing. Please resubmit a copy.

Pell Lifetime Limit-Cleared (FinAid) (NO ACTION REQUIRED BY STUDENT): This document will be removed by the Financial Aid Office once the Pell lifetime history has been reviewed. The Pell Grant has a lifetime limit of 600%.

Pell Overpayment-Cleared (Dept of Ed) (ACTION REQUIRED BY STUDENT): To resolve your Federal Pell Grant Overpayment, call the U.S. Department of Education at 1-800-621-3115, or write to the U.S. Department of Education, P.O. Box 5609, Greenville, Texas 75403-5609.

Perkins Entrance Interview (ACTION REQUIRED BY STUDENT): Once you've accepted a Federal Perkins Loan, you must complete loan entrance counseling and learn about the terms and conditions of your loans.  Students will need to complete counseling if they have never participated in the federal Perkins Loan program or have transferred to Louisiana Tech University from another school where they had a Perkins Loan.  The Entrance Counseling must be completed before the loan will be credited to your account.

Perkins Exit Interview (ACTION REQUIRED BY STUDENT): Any student who borrowed a Perkins Loan at Louisiana Tech University is required by law to complete a Perkins Loan Exit Interview if, at any time, the student is not enrolled at least half-time, graduates, or leaves the University (resignation, unofficial withdrawal, transferring to another school, etc).

Perkins Master Promissory Note (ACTION REQUIRED BY STUDENT): Once you've accepted a Federal Perkins Loan, you must complete the Master Promissory Note (MPN) in order for the loan to be credited to your student account.  When you sign the MPN, you are confirming that you understand that your school may process new loans on your behalf for the duration of your education (up to 10 years) without having you sign another MPN. You are also agreeing to repay all of the loans made to you under the terms of the MPN. Therefore, it is very important that you completely read and understand all of the information before you sign it.

Request for Dependency Override (STUDENT INITIATED ACTION): The student may inquire about the possibility of being independent, if there are unusual or extenuating circumstances that may be reviewed by our office.  The form would need to be requested to be sent to the student (must re-affirm each year) or student may pick up form in our office, if he/she is in the local area.  Along with the completion of the form by the student, supporting documentation must be provided and returned to the financial aid office.  Student will receive notification by email of the result of request.  If the result is favorable, there is no action taken by the student.  The financial aid office will submit electronically, a professional judgment of "dependency override" (FAFSA, of course must be on file as having been completed by the student regardless, if the student's FAFSA was/is rejected.

Request for Special Circumstances (STUDENT INITIATED ACTION): For 2017-2018, a family's 2015 income is used to assess financial need according to federal laws and regulations.  If a family's 2016 income is going to be lower due to "special circumstances" (such as loss of job, medical expenses not covered by insurance, one-time payment of a lump sum from a source, separation/divorce after FAFSA was filed, death of a current spouse, etc.)  You may contact financial aid either by email or phone for an inquiry of circumstances to determine if it is necessary/possible for any positive change to occur.  This document may be mailed to the student, after it has been determined that student warrants such a request. *Please note that student may request this form by stating on award letter, "possible modification" due to whatever the circumstances he/she feels need to be reviewed".  Supporting documentation must be provided.

If the student has received notice from the graduate school that they are unconditionally admitted, please contact the Financial Aid Office so that their graduate admission status can be confirmed.

Veterans Status (DD214) (ACTION REQUIRED BY STUDENT): You can obtain separation documents (usually called a DD-214) from The National Archives. The process is rather simple and for most cases it takes 3-4 weeks (or less).  You can request your documents either on-line or by mail. You can download the paper application here.  The online application, eVetRecs, will only work with Internet Explorer. Visit this page, click 'Launch the eVetRecs system to start your request Online' and answer the questions. When you're done, print, sign and either fax or mail the signature page to the address listed on the request form.
